2.After obtaining the "Physician Practice Certificate" or obtaining the title of physician, he has been engaged in clinical work in the same specialty in a medical institution at or above the first level for more than 3 years; or after obtaining the title of doctor, engaged in clinical work in the same specialty for more than 8 years in a medical institution above the first level;

3.Have a permanent residence certificate and resident ID card within the administrative area of the county (city);

4.After the opening examination of the municipal people's zhengfu health administrative department, the certificate of qualification was obtained.

Fourth, the application and approval procedures.

1.Individuals and units apply to open clinics and submit applications to the District Health Bureau.

2.The district health bureau accepts eligible applications and evaluates their medical technology qualifications, practice conditions and feasibility analysis reports.

5.The district health bureau shall, within 30 days from the date of accepting the application for practice registration, review and on-site inspection of the preparation situation, and conduct on-site spot checks and assessments of the basic knowledge and skills of relevant practitioners such as disinfection, isolation and aseptic operation, and issue the "Medical Institution Practice License" to the institutions that pass the acceptance; If the examination is unqualified, the reasons shall be explained in writing and the applicant shall be notified.

5. Submit the following materials:

1.Application for Establishment of Medical Institutions;

2.Feasibility analysis report;

3.site selection report;

4.Curriculum vitae and relevant documents of the main person in charge of the medical institution, including ID card, graduation certificate, professional title certificate, physician qualification certificate, physician practice certificate, certificate of enrollment (internal medical institution) or certificate of non-employment and family planning certificate (copy to be submitted after verification);

5.Proof of the qualifications of the legal person and the legal representative.

It is estimated that you cannot use your own practice certificate to apply for a clinic license, and you also need to hire an external doctor to sit for the consultation, because you cannot change the registration location of your practice certificate from your current employer.
