Huang Zhong was promoted to general for his merits, and was tied with Guan Yu, Zhang Fei, Ma Chao, and general until he died of illness in the twenty-fifth year of Jian'an (220).
In the Romance of the Three Kingdoms, Liu Bei and Wu avenged Guan Yu, and Huang Zhong ignored his old age and asked Ling to be the vanguard. Later, because Liu Beiyan's veteran was useless, Huang Zhong went out single-handedly, and on the way to pursue Wu general Pan Zhang, he unfortunately fell into Ma Zhong's ambush and was shot in the shoulder socket by him with a stray arrow. The cause of Huang Zhong's death is also somewhat controversial, according to the Romance of the Three Kingdoms, Huang Zhong was one of Liu Bei's Five Tiger Generals, who once killed Cao's general Xiahou Yuan in the Battle of Dingjun Mountain, helping Liu Bei capture Hanzhong. Later, he was shot by an arrow while pursuing the enemy general, Ma Chao, and eventually died of an arrow wound.
