The 5why analysis method, also known as the "5 questions method", is to ask yourself 5 "whys" in a row to investigate the root cause of a problem. Although it is 5 whys, it is not limited to only do "5 times why", mainly until the root cause must be found, sometimes it may only be 3 times, sometimes it may be 10 times, as the old saying goes: break the casserole and ask in the end.

The key to the 5 why method: Encourage problem solvers to try to avoid subjective or conceited assumptions and logical traps, start with the result, follow the chain of cause and effect, and find the root cause of the original problem.

5Why is implemented at three levels:1. Why does it happen? From the point of view of "manufacturing".

2. Why didn't you find it? From the point of view of "inspection".

3. Why is there no systematic prevention of accidents? From the point of view of "system" or "process".
